• Business name and DBA (if any)
• Business address and shipping address
• Contact name, phone, and email
• Resale or tax exemption certificate (US/Canada)
• A brief description of your studio or business
• Approximate annual dancewear spend

What happens next

We’ll review your application within 1–2 business days and respond by email. Approved accounts receive a welcome packet with line sheets, current pricing, payment terms, and ordering instructions.
